We are seeking a compassionate, reliable, and professional Home Health and Personal Care Aide to provide essential support to clients in their own homes. In this role, you will play a critical part in helping individuals maintain their independence, dignity, and quality of life by assisting with activities of daily living (ADLs), medication reminders, and companionship. Key Responsibilities
Direct Care & Assistance: Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and mobility support.
Health Monitoring: Monitor client health status, including vital signs as directed, observe changes in condition, and report any concerns immediately to supervisors or family members.
Medication Assistance: Provide medication reminders to ensure clients take prescribed doses on schedule, following established care plans.
Nutritional Support: Prepare and serve nutritious meals according to dietary guidelines and client preferences; assist with feeding if necessary.
Household Support: Perform light housekeeping tasks, including laundry, cleaning of living areas, and ensuring a safe, clutter-free environment.
Companionship & Engagement: Provide meaningful social interaction, emotional support, and assistance with recreational activities to promote mental and emotional well-being.
Record Keeping: Maintain accurate, timely documentation of care provided, daily activities, and any observed health changes in accordance with agency policy.
Compliance & Safety: Adhere strictly to infection control protocols, HIPAA privacy standards, and agency policies regarding professional boundaries and client rights.
Requirements & Qualifications
Education/Certification: High school diploma or equivalent. Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) is highly preferred and may be required based on state regulations.
Experience: Previous experience in home care, long-term care, or assisted living is desirable.
Key Skills:
Strong knowledge of basic hygiene, safety, and personal care techniques.
Ability to follow detailed care plans, prioritize tasks, and respond calmly to emergencies.
Excellent interpersonal and communication skills, with a patient and empathetic attitude.
Requirements:
Valid driver’s license and reliable personal transportation (if required to travel between client homes).
Ability to pass a comprehensive background check and health screening (including TB testing).
Physical ability to perform tasks such as lifting, transferring, or assisting clients with mobility (per safety guidelines).
Flexibility to work varying shifts, including weekends and holidays, as needed.
